Why TrueBlueMD
TrueBlueMD provides remote patient monitoring (RPM) and chronic care management (CCM) for family medicine practices. Many of your family medicine patients may already qualify. We supply the clinical support, connected devices, software, documentation, and monthly billing reports, so the program runs without pulling your staff away from patients.
A booth visit takes 15 minutes. The walkthrough takes 30 minutes, online. There's no slide deck, just a straight answer on what the program's worth to your panel.
Remote care programs
RPM and CCM designed, launched, and managed for your practice, from enrollment to the monthly report.
Experienced clinicians
Dedicated clinical support works as an extension of your team. Your physicians keep every clinical decision.
Digital health data
Connected devices send readings from home, so your team sees what happens between appointments.
Why programs stall
Hypertension, diabetes, heart failure, and COPD. Family medicine panels are full of patients who qualify for RPM and CCM. What most practices lack is the capacity to run it.
Both programs depend on documented time with every enrolled patient, every month. Your MAs and nurses are already at capacity.
Choosing, programming, shipping, and replacing devices, then teaching patients to use them, is a job of its own.
Consent, time logs, and monthly reports have to be right for every patient, every month, or claims won't hold up.
We take all three off your plate.Clinical support, connected devices, software, and the billing reports behind them, run by one team.

What we run
RPM and CCM for family medicine from a single partner, with the people, devices, software, and reporting included.
Devices patients actually use
Devices come pre-programmed and ship straight to the patient's home. Cellular models send readings without any Wi-Fi setup.

How it works
We handle the operational steps that make RPM and CCM hard to run in-house. Anything that needs clinical judgment comes back to your team.

We review your chronic panels and flag priority groups: hypertension, diabetes, obesity, COPD, CHF, and patients with several conditions.
We explain the program to each patient, enroll them, and ship pre-programmed devices to their home with setup support.
Built for each patient
Every patient gets a care plan built around their conditions and goals, and approved by your physician. Monthly check-ins, wellness calls, and coaching keep them on track, with updates flowing back to your team.
Our team reviews readings between visits against each patient's care plan, with clear follow-up and escalation rules.
You get evidence of care and monthly billing reports, ready for your billing team.

No. Our clinical support team handles enrollment, patient touchpoints, readings follow-up, and documentation. Your physicians stay in charge of clinical decisions and escalations.
No. We run programs for independent practices, multi-provider groups, ACOs, and community clinics that want remote care without building every workflow themselves.
Practices usually start with Medicare patients living with hypertension, diabetes, obesity, COPD, CHF, or several chronic conditions at once.
It depends on patient volume and which programs you run. Pricing is part of the conversation, along with what your practice keeps, so you leave with both numbers.
