Patients who were hard to reach
Before TrueBlueMD, many of First Fruits Medical Clinic’s patients were not easy to reach between visits. The two-provider clinic cares for patients living with hypertension, diabetes, heart failure and obesity.
Initially it was not easy to reach many of our patients until TrueBlueMD came in.
Two programs, run end to end
In July 2023, on a recommendation, the clinic launched Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) with TrueBlueMD. TrueBlueMD runs both programs end to end, from patient onboarding and connected devices to monitoring, patient outreach and alert escalation. The clinic handles billing, and its providers make the clinical decisions.

Connected blood pressure monitors, glucometers and scales send readings from patients’ homes, and concerning readings are escalated to the care team.
Chronic Care Management
Ongoing care management for patients living with chronic conditions, run by the same TrueBlueMD team.

Since launch, the clinic says patient care, staff workload and revenue have all improved, and its monitored patients stay engaged between visits.
Monitored patients stay engaged
83% of monitored patients send readings on at least 16 days a month, and around 4 alerts a month are escalated to the care team so providers can act early.
