In August 2022, Primary Care Medicine launched Remote Patient Monitoring with TrueBlueMD. Today, 400 patients are enrolled, and the care team sees their home readings between office visits.
Office Manager Kayla Cason shares what changed for the care team, the staff and the practice.

Little visibility between office visits
Between office visits, the care team had limited visibility into how patients with chronic conditions were doing at home. Getting timely home readings, spotting concerning trends early and keeping patients engaged in their own health were all difficult.
We struggled with consistently monitoring our patients between office visits, especially those with hypertension, diabetes, and other chronic conditions.
An RPM program run end to end
In August 2022, the practice launched Remote Patient Monitoring with TrueBlueMD, which runs the program end to end: patient onboarding, connected devices, ongoing monitoring, patient outreach and alert escalation. The practice handles billing, and its providers stay focused on clinical decisions.

Visibility for the care team, a new revenue stream for the practice
For the care team, the biggest change is visibility. Instead of waiting for the next office visit, they see patients’ blood pressure, glucose and oxygen readings as they come in, and they hear about concerning trends early. The program has also become a new revenue stream for the practice.
Patients stay engaged between visits
62% of enrolled patients send readings on at least 16 days a month, and around 10 alerts a month are escalated to the care team.
